Diabetes Self-Management Training (DSMT) Program

DSMT is a comprehensive program that provides the knowledge and skill needed to manage your diabetes. A referral by your physician or primary care provider is required.

Diagnosis of Diabetes
Fasting blood glucose >126mg/dl.

  • This should be confirmed by repeat testing on a different day unless obvious symptoms of diabetes exist. Symptoms may be:
  • Excessive thirst
  • Increased urination
  • Unexplained weight loss
  • Blurred vision

Random blood glucose >200 mg/dl plus symptoms of diabetes.

  • Confirmed by repeat testing on different day.

2 hour plasma glucose > 200 mg/dl during an oral glucose tolerance test (OGTT).

Our Program Goals
Provide diabetes education to all clients referred with new onset or uncontrolled diabetes
Improve blood sugar control
Delay or prevent complications of diabetes such as:

  • Foot ulcers and amputations
  • Renal failure
  • Blindness
  • Heart disease and stroke
  • Improve quality of life for clients

Session 1 – Initial Evaluation
Individual session with RN that focuses on areas related to diabetes and your learning needs.

  • An overview of diabetes: types of diabetes, how your body uses glucose and insulin, introduction to carbohydrate counting, medications
  • Development of personal goals based on American Diabetes Association (ADA) guidelines
  • Foot exam, plus instructions for daily foot care
  • Blood glucose testing: how to use your meter, calibration, coding, getting a blood sample, when and why to do testing, identifying patterns and setting individual goals
  • Communicating with your doctor

Session 2 – Nutrition Therapy
Group session with a Registered Dietitian/Diabetes Educator

  • Meal Planning Guidelines
  • Carbohydrate Counting
  • Label Reading
  • Dietary Fat & Cholesterol Modification
  • Role of Fiber on Blood Sugar & Cholesterol Control

Session 3 - Diabetes Management
Group session with Diabetes Educator

  • Self-monitoring plus pattern identification and management
  • ADA guidelines
  • Development, prevention, detection, and management of long term complications
  • Prevention and treatment of low and high blood sugar
  • Sick Day Management Plan, when to call your doctor
  • Stress
  • Travel
  • Lifestyle changes and action plan

Session 4 - Diabetes & Exercise
Group Session with an Exercise Physiologist and Diabetes Educator

  • Exercise options and benefits
  • Exercise guidelines, precautions and progression
  • Methods for monitoring intensity
  • Discharge plan and follow up

OTHER PROGRAM COMPONENTS

  • Additional individualized session for insulin teaching
  • Insulin self-adjustment training available as recommended by your doctor
  • Follow up telephone contact after program completion
