Cardiac Wellness

Cardiac Wellness, formerly known as Cardiac Rehab, is a secondary prevention program that educates people about reducing the risk of future cardiac events through exercise, nutrition, and heart healthy information. Most commonly this service is provided after a heart attack, open-heart surgery, or diagnosis of stable angina. Additional diagnoses do apply based on individual insurance plans. The Cardiac Wellness area is staffed by a Registered Nurse and an Exercise Physiologist with experience in cardiac care.

An individualized care plan is created with staff guidance based on an individual's goals. The person must commit to working on this care plan three day’s per week. The plan of care may be four weeks up to three months long, depending on the goals that are set and progression toward meeting those goals. Informal education is a part of each session and formal education is provided one time per week in a variety of ways to appeal to different types of learners. The individual, family and/or support system verbalize improved confidence after attending sessions. By working together as a team, the changes made can last a lifetime.
